Job Description:

Reporting to the Community Programs Manager, you will be leading the delivery of caregiver support services in York Region and South Simcoe. Working within an integrated wellness model, you will provide support to caregivers, both individual clients and in groups. Services include: wellness education, caregiver counselling and information to maximize co-ordination and access to services and meet the support needs of the caregiver.

  • Experience working with a multidisciplinary team environment, with excellent assessment, communication and problem-solving skills.
  • Demonstrate presentation, teaching and coaching skills
  • A diploma in Social Service Work with current membership with the Ontario College of Social Workers and Social Service Workers, combined with minimum 3 years experience providing senior and/or caregiver support services in the community including program development.
  • Strong computer skills with proficiency in word processing and spreadsheet applications are required.
  • A second language will be considered an asset.
  • A valid Ontario driver’s licence and a reliable vehicle are required.
